Reading the entire form ensures the caregiver understands: (1) who we are, (2) the purpose of participating in EPW, including this research study, and that this participation is voluntary (3) this research study, (4) how we respect their privacy and confidentiality, (5) has a chance to consent for their child to participate in EPW, including this research study, (6) has the chance to ask us questions and have them answered; AND THEREFORE CAN MAKE AN INFORMED DECISION ABOUT PARTICIPATING AND HAVING THEIR CHILD PARTICIPATE.

    First, during the caregiver survey, a child's caregiver decides whether or not to consent for their child to participate. Then, IF the caregiver consents for their child to participate, you can start the student survey with the child, asking the child whether or not he or she assents to the interview. The interview only continues if the child assents; but, the child can stop the interview at any point in time.

    Be neutral and take time to sincerely thank the caregiver for her time and feedback.     Always be friendly and respectful. Introduce yourself. Give regular eye contact. Read the informed consent/assent scripts aloud word for word. Report any issues to your supervisor, including if a child discloses he/she has been harmed. Conduct the interview in a semi-private area where the respondent will be comfortable and also within sight of others. Allow the respondent a short break nearby if needed. Be neutral but engaged. Ask questions exactly as they are worded. Ask ALL questions in the survey. Read aloud all parts of the survey that are indicated. Follow instructions. Do not skip the sections of the script that introduce the section of questions. At the end of the interview, thank the respondent, ask them if they have any questions, and address any questions they have.
